ODA - (Oracle Database Appliance) DescriptionDisks may randomly show as physically missing or in predictive failure after a disk replacement or reboot while using ODA OAK patch level 2.1.0.3.0 OccurrenceThere is a high possibility after replacing a disk or if you encounter a crash while on OAK / ODA patchset level 2.1.0.3.0 that you can encounter this problem You must be on the Oracle Database Appliance (aka ODA) You must be using 2.1.0.3.0 Please refer to note 888888.1 Oracle Database Appliance - 2.X Supported Versions & Known Issues @ this is a cloned note of ALERT - Urgent Mandatory OAK Patch 2.1.0.3.1 (Doc ID 1438089.1) SymptomsDisks may show as randomly unavailable, missing or in predictive failure If you bounce the database or reboot only one of the two ODA servers the problem can still be encountered for the same or different disks WorkaroundRecycling / power down of _both_ of the ODA Servers will relieve the problem. Patches2.1.0.3.1 was created to fix this problem 2.2.0.x does not have this problem Obtain OAK Patch 2.1.0.3.1 from the My Oracle Support (MO) website as Patch 13817532.
