Cause

After doing the bare metal restore with 2.1.0.3.1 image, tying to boot(reboot) the system.

Solution

> Power cycle the node using ILOM (if you are not near the server)

You can login to the ILOM
Go to Remote Control -> Remote Power Control
From the drop down list select 'Power Cycle'

> Now open the remote console

Go to Remote Control -> Redirection
   Launch Remote Console

> When it shows you the grub boot line
Booting Oracle Database Appliance (2.6.18....) in 3 seconds....
   Press  'e' over here.

> Now it will show following lines:
   Oracle Database Appliance  (2.6.18....)
Use arrow key and bring the cursor to this line

> Again press 'e' to edit it and bring the cursor to line
   kernel /vmlinuz....

> Again press 'e'

> Now it will bring you to a prompt

> just add 'nousb' at the end of this line and hit enter

> After this it will come to the previous screen and hit 'b' this time

> After this this should boot successfully

> Once the system boots fine, edit the /etc/grub.conf to have nousb added so that next time you do not need to repeat above procedure.
