Symptoms

Messages in the error warning info event log:

- Error from device Code: 605 - Drive media error, drive 2,1,-1,1,1: load FAILED with mediaError: , FSC: 5135

- Warning from device, drive 2,1,-1,1,1: rewindUnload Successful with mediaError: , FSC: 5135

- Error from device Code: 601 - Drive is loaded, drive 2,1,-1,1,2: rewindUnload FAILED with unitCheck 495E - drive is loaded ()

- Error from device Code: 601 - Drive is loaded, drive 2,1,-1,1,2: rewindUnload FAILED with unitCheck 5132 - drive is loaded ()

Cause

1. The tape leader itself was 'pulled into' the tape and the drive could not 'grab' it on a load because it was not in the 'normal position'.

2. Another tape was physically broken and stuck inside the drive. The drive has to be replaced.

During the load sequence, slack in the tape cartridge could not
be removed in the allotted time. Tape breakage or a jam within the
cartridge is a possibility.

The servo interface task received a command that requires tape tension. Unfortunately we are in a state trying to recover the tape tension, so the command could not be performed. Issue an unload to get the tape out of the drive and retry with another tape.

Solution

If  broken tape or a broken tape leader:
    - Check to see if other tapes load in the drive.
    - See if the drive will unload.
       - If it fails to unload then it is probably a broken tape.
            - If the tape is broken, you will need to splice the tape in order to wind it back into the cartridge.
              (Scotch tape can be used to splice)
            - If the tape is broken, contact Oracle Hardware Support for data recovery options should there not
              be other copies of the data.

If his tape has the leader pulled inside the cartridge:
    - Eject the cartridge from the tape drive.
    - Pull open the cartridge window and verify whether the cartridge loop is seated in the correct position.
    - If the leader is not properly seated in the cartridge window, a field engineer with access to a T10k chuck
       tool can fix the tape cartridge in under a minute.

If other type of tape damage exists, contact TSC tape support via MOS.
