Solution

Change HBA driver:
Customer have T10000 drives connected to a server running Windows 2003 SP1.

The tape drives disappear from the server after a period of heavy use. The drives are connected directly to the server. They are not going through a switch.

The customer is using Emlux LP11002 4gb HBAs at firmware level 250A6 and driver level 1.20A7, which is the next to the latest version for both the firmware and drivers.

Customer changed the HBA drivers from the Storport driver to the FC port driver. This appears to have resolved the problem.
