Issue Description:
During heavy disk usage,Sun Fire 15K domains containing SunSwift
PCI SCSI/ethernet combo adapters, also known as Fresh Choice,
may encounter SCSI errors.
Here is a sample error:

Affected SunSwift PCI adapters have a beginning date code of 0203
and serial numbers starting with 5015656067xxx. The date code and
serial number can only be determined by a visual inspection of the
adapter.
Manufacturing Operations purged all suspect adapters before any
were shipped in systems via Stop Ship Purge P001-20037.A1.
However, there is some potential that a small number may have
made their way in to FRU stock locations and shipped via xoptions
(If the sample error message above is encountered) the domain containing
the (X)1032A SunSwift PCI adapters will generate write errors for the
target device. These errors are retryable and generate "reducing
transfer rate" messages in the domain's /var/adm/message.log.
Root cause determined a UCC terminator component on the board was
generating excessive noise, which led to these SCSI errors.
Corrective Action was made available via ECO W0_24292 on June 04, 2002
by releasing part number 501-5727-03.

Corrective Action:
The chance of this problem occurring is very small. However, if
F15K domains containing SunSwift PCI SCSI adapters experience
SCSI performance issues, including "reducing transfer rate"
error messages, the (X)1032A adapter should be examined for the
following:
1. P/N 501-5656-05
2. Date Code starting from 0203
3. Beginning serial number of 5015656067xxx
If the 3 conditions above are true, replace with Single-Ended
Ultra/Wide SCSI (SunSwift PCI) p/n 501-5727-03.
The system should be checked for the presence of any such device. From
any login session into an F15K domain issue the following command:
# prtconf |grep isp
SUNW,isptwo, instance #0 <<<< one adapter present

Billing Type:
Warranty: Sun will provide parts at no charge under Warranty
Service. On-Site Labor Rates are based on how the
system was initially installed.
Contract: Sun will provide parts at no charge. On-Site Labor Rates
are based on the type of service contract.
Non Contract: Sun will provide parts at no charge. Installation by
Sun is available based on the On-Site Labor Rates
defined in the Price List.

i) In case of Mandatory FCOs, Enterprise Services will attempt to contact
all known customers to recommend the part upgrade.
ii) For controlled proactive swap FCOs, Enterprise Services mission critical
support teams will initiate proactive swap efforts for their respective
accounts, as required.
iii) For Replace upon Failure FCOs, Enterprise Services partners will implement
the necessary corrective actions as and when they are required.
