Issue Description: 
At system power up on an Ultra Enterprise E4x00/5x00/6x00, the disk
board (Sun p/n 501-3113-02) does not turn on.  This problem may be seen
in the field if one of the following two scenario's occur:

  1. A disk board is added to a system with a pre-existing disk board.

  2. System is upgraded by adding other modules (CPU, I/O boards, etc).

POST will fail with 'SCSI_VDD' error message in DIAG mode.

Root cause has been attributed to a load performance issue with the FDK
42W DC/DC converter (300-1109-02) on the Internal 4.2GB Disk Board.
This component part has been dash rolled to a -03 which adds a resistor
into the circuit.  The corrective action was implemented into
Enterprise Services Logistics via LEAP# 948 in late July 1998 based on
ECO# WO_13281 (released on June 5, 1998), which rolled the dash on the
Internal 4.2GB Disk Board from 501-3113-02 to 501-3113-03.

Corrective Action: 
Upon failure replace 501-3113-02 with 501-3113-03.
Billing Type: 
Warranty: Sun will provide parts at no charge under Warranty 
           Service. On-Site Labor Rates are based on how the
           system was initially installed.

 Contract: Sun will provide parts at no charge. On-Site Labor Rates
           are based on the type of service contract.

 Non Contract: Sun will provide parts at no charge. Installation by
               Sun is available based on the On-Site Labor Rates 
               defined in the Price List.

i)   In case of Mandatory FCOs, Enterprise Services will attempt to contact
      all known customers to recommend the part upgrade.

ii)  For controlled proactive swap FCOs, Enterprise Services mission critical
     support teams will initiate proactive swap efforts for their respective 
     accounts, as required.

iii) For Replace upon Failure FCOs, Enterprise Services partners will implement 
     the necessary corrective actions as and when they are required.
